About the Role

PURPOSE OF THE ROLE

The purpose of the Mentor role is to directly support their allocated clients and have clear oversight of the client's progression cycle. The Mentor will actively assess their clients' support needs driving a clear pathway towards the client's independence. The Mentor liaises closely with the Deputy and Operational manager roles within Monthly Review Meetings, and being able to be decisive and clear in communicating changes, Support review outcomes, and attending assessments with either the Operational or Deputy manager.

SUMMARY OF DUTIES

The Mentor Duties include the oversight of up to 8 client contracts. Within their Role the Mentor will be responsible for the dynamic assessment and ongoing support provided to their clients. Within this they will record weekly Progression Reports and review the support for their clients periodically.

The Mentor is responsible for having a clear oversight of the support provided to their allocated clients and utilising their Support Plan to track, progress, and establish strategies for both how the client themselves, and our Team can best support them to achieve independence.

The Mentor will promote, inspire, and demonstrate Trust by maintaining clear communication with in the team and during Monthly Review Meetings with the Team.

The Mentor's specific duties will include, but not be limited to:

Supporting their Assigned Clients
Covering Support Sessions with Un-Assigned Clients
Completing, Maintaining, and Updating their Assigned Clients' Support Plans with their Client
Completing, Maintaining, and Updating their Assigned Client's Individual Risk Assessment
Planning Activities
Acquiring / Completing and Submitting Risk Assessments for specific Activities ( Operations Manager or Deputy Manager Sign off required)
Keeping up to date with Policy reviews, New Policies/Procedures, General Risk Assessments, and Amendments.
Upkeeping Employee Health & Safety at Work duties and responsibilities
Attending all required Training
Keeping all held records (i.e. Vehicle details, insurance, and Driving License Information) up to date
Recognising, Responding to, and Reporting Safeguarding Concerns, Incidents, Accidents Appropriately
Completing Weekly Progression Reports
Attending and Actively Partaking in Monthly Review Meetings
Attending and Actively Partaking in Monthly Supervision Attending and Actively Partaking in Annual Appraisals
Co-Developing of Support Tools, Services, Workshops, and Strategies available within the Team
Assessing New Referrals with the Operations Manager or Assistant Manager
Attending Multi-Agency Meetings and Advocating for your assigned Clients
Attending Professional Multi-Agency Reviews for your assigned Clients

Requirements

Education:

  • GCSE or equivalent (required)


Experience:

  • Support: 2 years (preferred)

  • key working or Life Coaching: 1 year (preferred)

  • Working with Neurodiverity: 1 year (preferred)


Language:

  • Fluent English (required)


Licence/Certification:

  • Driving License (required)
